Isolated Undersized Mitral Annuloplasty for Functional Mitral Regurgitation in Non-Ischemic Dilated Cardiomyopathy

چکیده
he appearance of functional mitral regurgitation (MR) predicts poor survival for patients with nonischemic dilated cardiomyopathy (DCM). Therefore, surgical treatment is recognized as crucial for improving life expectancy. Regarding mitral reconstruction in DCM, Bolling et al reported the clinical usefulness of the undersized mitral annuloplasty (MAP) in 1995;1 however, recurrent or persistent MR after annuloplasty has been reported.2–4 Regarding recurrence of MR, Calafiore et al reported that DCM patients with a coaptation depth (CD) ≥11 mm were likely to have persistent MR after mitral valve (MV) repair. 2

Evaluation of Subvalvular Apparatus by 2-D Transthoracic Echocardiography
Functional mitral regurgitation (MR) is one of the major contributing factors for heart failure and hospitalization in both nonischemic and ischemic dilated cardiomyopathy. The limitation of undersized mitral annuloplasty has been recognized. Recurrent late MR is associated with continued left ventricular (LV) remodeling and enhanced papillary muscle displacement outside posterior ring after mi...
متن کاملMitral valve repair for functional mitral regurgitation in end-stage dilated cardiomyopathy: role of the "edge-to-edge" technique.
BACKGROUND The aim of this study was to assess the results of mitral valve (MV) repair in functional mitral regurgitation because of end-stage dilated cardiomyopathy (DCM). METHODS AND RESULTS Seventy-seven patients with end-stage idiopathic (26 patients) or ischemic (51 patients) DCM underwent MV repair for functional mitral regurgitation (3 to 4+/4+). Fifty-eight patients (75.3%) were in Ne...
متن کاملSurgical treatment for functional mitral regurgitation secondary to dilated cardiomyopathy: Current options and future trends
There is an increasing number of patients with mitral regurgitation secondary to dilated cardiomyopathy. Recent data suggest that mitral regurgitation (MR) can be surgically corrected in heart failure with symptomatic improvements and favourable reverse left ventricular remodeling. However, several questions remain to be answered, regarding the optimal management of functional mitral regurgitat...
متن کاملProsthetic Ring Endocarditis with Shrunken Mitral Annulus Late After Undersized Mitral Annuloplasty
We report a case of supra-annular mitral valve replacement for a patient with prosthetic ring endocarditis that developed late after undergoing undersized mitral annuloplasty (UMAP). A 58year-old man had undergone left ventriculoplasty and UMAP with a 26-mm semi-rigid full ring for dilated cardiomyopathy and functional mitral regurgitation. The mitral valve had progressively stenosed and he dev...
متن کاملThe extent of early left ventricular reverse remodelling is related to midterm outcomes after restrictive mitral annuloplasty in patients with non-ischaemic dilated cardiomyopathy and functional mitral regurgitation.
OBJECTIVE Restrictive mitral annuloplasty (RMA) can often improve heart failure symptoms and induce left ventricular (LV) reverse remodelling in patients with non-ischaemic dilated cardiomyopathy (DCM) and functional mitral regurgitation. However, it is unknown whether the observed LV reverse remodelling translates into better outcomes or not. METHOD Fifty patients with advanced non-ischaemic...
